Orientation Forms are now available online through PowerSchool. In order to complete them, you will need to sign in at https://powerschools.nhboe.net/public/.  You must have an established account and then link your child to the account. If necessary, please email Mrs. McGovern for your PowerSchool parent access information at patricia.mcgovern@nhboe.net. 

There are four (4) mandatory forms. Please be aware that before a student enters a NHPS building or receives a device, you must complete the forms for each student in your family.

Forms can be found on the lefthand side of your PowerSchool homepage under Navigation.

Please write the password down and keep it in a safe place.

Physical Assessment Requirements

Please be aware that your child will not be allowed to attend school if the following requirements are not met.  

Forms can be faxed to the school at 475-220-3605 or dropped off at The Bureau of Nursing Office, 54 Meadow Street, 1st Floor.
 
Pre-Kindergarten: An up to date physical exam must be entered on a State of Connecticut Health Assessment form dated within one year of the student's entry. 
 
Kindergarten: An up to date physical exam must be entered on a State of Connecticut Health Assessment form dated within one year of the student's entry. 
 
7th grade: Students entering 7th  are also required to have an updated physical exam and immunizations per Connecticut state law.    

Students who do not present these records will not be allowed to attend school.
